Treatment Plan

Writing a treatment plan can be really easy and it can be a thing you can overthink and get lost in. Rule of thumb, find three things the client can work on, tasks to match and then set them to dates for completion. Thats it!

Treatment (goal) Plan Template

... a goal without a plan is just a wish

Here you will describe what do you want to accomplish? e.g. to stay sober for 30 days
Here you will identify how long you think it will take for you to accomplish the objective e.g. 1 month, start date 1/1/24 to end date 2/1/24
Here you will describe what kind of goal you would like to accomplish in order to complete your main objective. e.g., Attend multiple community recovery groups, find a "home" group
Here you will describe the first step to accomplishing this goal, e.g., identify recovery group options
Here you will describe the second step to accomplishing this goal, e.g., locate recovery groups in your area
Here you will describe the Third step to accomplishing this goal, e.g., Schedule dates and times you will attend some groups
Here you will describe the fourth step to accomplishing this goal, e.g., Attend some groups

Repeat these steps as many times as you need in order to help you or the client develop a plan for MH or addiction recovery treatment
