4. Address

5. Practice Name (group or individual)

6. NPI

   1. If billing as a group just practice NPI

   2. NPI for each practitioner

7. Group Practice or individual  tax ID​

8. Log ins to EHR, Availity, Change, Medicaid, & all insurance portals you are credentialed with or ewant to be credentialed with.
